Person-Centered Planning (PCP) is a transformative approach that empowers individuals to take an active role in shaping their lives, ensuring that services and supports align with their personal goals, preferences, and needs. This presentation explores the fundamental principles, benefits, and best practices of PCP, emphasizing the importance of balancing what is “Important To” and “Important For” to an individual. Through real-life examples, key strategies, and Jordan sharing his People Planning Together Plan, participants will gain insights into fostering a more inclusive, strengths-based, and self-directed planning process. Participants will leave the webinar with information to help develop an individual plan they can use to guide conversations with their families, friends, or support teams/circles in their day-to-day lives.
